What counts as a hallucination or bad answer?
Any agent output that is factually wrong, inconsistent with your data, harmful, or fails to meet user intent. This includes confident-sounding false statements, invented citations, bad code, incorrect medical or financial guidance, and policy violations.
How does detection method affect cost?
User reports are free but catch the fewest errors. Automated evals and guardrails add per-output cost but catch most issues early. Human audit is the most expensive but has the highest coverage. The calculator adds the detection cost and scales caught-error remediation by coverage.
Does this include the cost of preventing hallucinations?

What is the compliance / fine component?
Severe bad answers in regulated domains carry a small probability of a fine or lawsuit. The calculator multiplies that probability by a typical average fine for the severity level. This is an expected value, not a prediction of any specific event.
